The little girl is the girl in the White Room on Angel. I made the icon based on a comment Russell T Davies made in the podcast commentary for "The Satan Pit". There was concern that they didn't have the money to create the Beast in CGI so some of the other options Russell T Davies came up with included having a little girl like in the White Room.
